Collaborative care (CC) is the treatment of choice for major depressive disorder (MDD) as well as MDD with chronic medical conditions in primary care. In the Depression Initiative, a national initiative to improve depression treatment in the Netherlands, CC has been implemented and evaluated in Randomised Clinical Trials(RCTs) in three treatment settings: primary care(PC)(CC:DIP), occupational health care(CC:DOC) and the general hospital(GH) outpatient setting(CC:DIM). It is unknown which setting is most cost effective to deliver CC. Aims of the study: Compare cost utility of CC versus Care As Usual (CAU) for patients with MDD in the three settings.
